Confidentiality

The Alzheimer’s Foundation of America (AFA) is committed to protecting the privacy, dignity, and rights of all individuals who participate in our research studies. All personal information, health data, and study responses collected through AFA-sponsored research are kept strictly confidential and used solely for scientific and programmatic purposes.

• Your data will be de-identified before analysis.
• All information you provide during the sign-up process will be kept confidential and secure.
• Your data will be stored securely, accessible only to authorized research team members, and will be reported in aggregate form without identifying any individual participant.
• AFA does not sell, share, or disclose personal information to third parties outside of the research process unless required by law.

Pre-Signup Agreement

Study Title: Exploring the Biopsychosocial Impact of Dementia Caregiving on Spousal Caregivers’ Health Outcomes.

Purpose of the Study: This nationwide research study seeks to understand how caring for a spouse or long-term partner living with dementia affects the spousal caregiver’s physical health, emotional well-being, cognitive functioning, and social experiences.

What Participation Involves

By agreeing to proceed, you acknowledge that:

• You are a spousal or long-term partner caregiver of a person living with Alzheimer’s disease or another form of dementia.attention, or executive functioning, highlighting the potential cognitive toll of sustained stress.
• You are 21 years of age or older and reside in the United States.
• You understand that participation is voluntary and includes completing one online survey, optional follow-up interviews, and providing information about your caregiving experiences, health, and daily stressors.
• You understand that you may withdraw at any time, for any reason, without penalty and without affecting any services or support you receive from AFA.

What This Agreement Does Not Do

This pre-sign-up statement is not a consent form and does not enroll you in the study. Instead, it ensures that you understand the nature of the study before moving forward. If you continue with the registration process, you will later be provided with a formal Informed Consent Form that you must review and sign before you begin any study activities.

Voluntary Participation

Your decision to continue is entirely voluntary. You may choose not to proceed, and this decision will not affect your relationship with the Alzheimer’s Foundation of America.
